1. Historical background

According to official documents, on January 18 (30), 1841, the Conference of the Medico-Surgical Academy approved the idea of ​​organizing a hospital surgery clinic at the Academy, as well as the proposal “On the appointment of a well-known not only in Russia, but also abroad for his excellent talents and art in operative surgery Nikolai Ivanovich Pirogov to the post of professor of hospital surgical clinic.

The archive of the Military Medical Museum of the Ministry of Defense of the Russian Federation (Pirogov's fund) contains the message of the Conference of the Academy No. 234 dated March 3, 1841 that N.I. The establishment of the department with the clinic was due to the need to introduce a new teaching method, which involved the study of various morbid forms by comparing a large number of similar cases and identifying their individual characteristics. This original method of training future doctors had no precedent in the past and has firmly entered the system of medical education in Russia. N.I. After N.I. Pirogov by the department was led by his students: from 1855 to 1858. – Peter Yulievich Nemmert (1818 – 1858), and from 1858 to 1870. – Alexander Alexandrovich Kiter (1813 – 1879). They continued the work of their teacher. During this period, the antiseptic method of D. Lister (1867) was first embedded in Russia.

Subsequently, famous professors were headed by the department: from 1870 to 1888. – Eustathius Ivanovich Bogdanovsky (1834 – 1888) and from 1890 to 1901. – Vasily Alexandrovich Ratimov (1830-1904). Thanks to the efforts of V.A. Ratimov’s clinic was one of the first in Russia in accordance with the requirements of aseptics and antiseptics, and surgical interventions began to be performed in the operating room in sterile conditions.

A large stage in the history of the department (1903 – 1936) is associated with the name Sergey Petrovich Fedorov (1869 – 1936) – an outstanding domestic surgeon, a label -media of his Imperial Majesty Nicholas II, Honored Worker of Science of the RSFSR, founder of bile surgery, urology and neurosurgery in our country. Scientific interests S.P. Fedorov was very diverse. One of the first in our country S.P. Fedorov introduced the methodology of rectum and sigmoscopy, as well as esophagoscopy and cystoscopy. S.P. Fedorov is a pioneer in the use of intravenous anesthesia in clinical practice.

Under his leadership, blood transfusion problems were developed in detail. The priority of the first blood transfusion, taking into account isogemagglutination in our country, rightfully belongs to S.P. Fedorov and his students. Great merit S.P. Fedorov in the development of surgery of the abdominal organs and retroperitoneal space. So, S.P. Fedorov performed the first gastroctomy in Russia. He developed the scientific and methodological foundations of the urological service, organized the Society of Urologists and was its permanent chairman. S.P. Fedorov is rightfully considered the founder of liver surgery and biliary tract, raising it to the level of achievements of Western scientists. No less important is the contribution of S.P. Fedorov in the formation of neurosurgery.

In 1928, S.P. Fedorov – one of the first scientists of our country, was awarded the title of Honored Worker of Science of the RSFSR, and in 1933 he was awarded the highest award – the Order of Lenin and his name was assigned to the clinic of faculty (hospital) surgery founded by N.I. Pirogov.

After the death of S.P. Fedorova became the head of the department Vasily Ivanovich Dobrothelsky (1869 – 1937). He headed the department in 1936 and led it for only one year, as he suddenly died in 1937 during a short managing department of prof. The hospital clinic became the Faculty of Dobravsky. This happened due to the following circumstances. From the memoirs of P.P. Goncharova (P.P. Goncharov “Essays on the history of the Military Medical Academy in the post-October period”, 1960).

“When the 5th year was abolished and four-year training was introduced, therapeutic clinics were combined (under the leadership of M.A. Arinkin), while surgical clinics retained independence, but lost their names (3rd year clinic, 4th year clinic). The reduction affected only the academic (faculty) surgical clinic, headed by V.A. The Oppel, which in 1918 switched to it from general surgery, took this last after V.A. Oppel S.S. Girgolav.

Instead of and on the site of the abolished faculty (former academic) clinic, a department and clinic of military field surgery was created, which was organized and led by V.A. Oppel (after his death – V.A. Pavlenko). When the five -year training course was restored, the clinics of general surgery (S.S. Girgolav) and faculty surgery were restored (S.P. The clinic of military field surgery remained in its place (Willy Clinic) until the Great Patriotic War, and after the war was posted on the basis of the district hospital. After the war, in connection with the reform of the Academy, clinics were created: General Surgery (N.N. Elansky), faculty surgery (P.A. Kupriyanov), Hospital surgery (S.S. Girgolav), neurosurgery clinics (V.N. Shamov) et al. V.N. Shamov insisted that he headed by the clinic (“Fedorov Clinic”) be called “Faculty of Surgical Clinic No. 1”, and the clinic, headed by P.A. The clinic of faculty surgery has not preserved any continuity – created by I.F. Bush (the first surgical clinic in our country), it continued by N.A.Velyaminova, then represented by his student I.E. Gagen-Torn, then V.A. Oppel (a student of Ratimov and R.R. Zalena), after which it was eliminated, and her story ended on this.
After the war, the Fedorovskoye Hospital Surgical School became the new faculty school and should continue the history of the faculty school, in fact, it is and rightfully considers itself the successor and successor of the Fedorov hospital school.

The clinic of hospital surgery was essentially the successor to the Oppel school in general surgery and did not inherit the stories of the Pirogov and Fedorovsky hospital clinic. ” The changes in the name of the department, but the continuity of the hospital clinic in its monograph, is also described by Professor Nechai A.I. In 1939, a student of S.P. Fedorova Vladimir Nikolaevich Shamov (1882 – 1962), who headed the department and led it from 1939 to 1958. With the activities of V.N. Since 1991, the next period began in the history of the department. This period is due to significant teaching transformations at the Academy and the periprofilization of almost all clinical departments of both surgical and therapeutic profile. The Department of Faculty of Surgery received the name of the Department of Abdominal Surgery, preserving the name S.P. Fedorova. Teaching is carried out in the interests of a certain section of surgery both in the faculty and hospital type of training, with an unchanged desire to develop clinical thinking from students when analyzing patients.At the same time, training is systematically conducted for surgeons who come to the academy to specialize in abdominal surgery.

From 1994 to 2018 the department was headed Nikolai Anatolievich Maistrenko – Doctor of Medical Sciences, Honored Doctor of the Russian Federation, Academician of the Russian Academy of Sciences, Professor, Member of the Academy of Military Sciences, Academy of Medical and Technical Sciences, Petrovsky Academy of Sciences and Arts, Academician of the Russian Academy of Medical Sciences, Military Medical Academy.

In 2000, the academy returned to the three-degree system of teaching the main clinical disciplines. By the decision of the command of the academy in 2002, the department was again renamed the department of faculty surgery named after. S.P. Fedorov.

In 2018, the department was headed by Corresponding Member of the Russian Academy of Sciences, Professor, Colonel of the Medical Service Romashchenko Pavel Nikolaevich, doctor of medical sciences, professor, corresponding member of the Russian Academy of Sciences, doctor of the highest qualification category.

Over the years of the existence of the department, many of its employees have become prominent professors and headed the departments of surgery, both at the academy and in other universities of the country. Seven professors were awarded prizes for scientific achievements: V.N. Shamov – the Lenin Prize, and V.M. Sitenko, A.I. Nechay, A.A. Kurygin, L.V. Lebedev (twice), S.V. Ryzhkov, B.A. Samotokin – State Prize. Two former employees of the department, who became independent leaders of scientific schools, were awarded the high title of Hero of Socialist Labor with the award of the Gold Star badge.   • The circle of the Military Scientific Society of cadets and students of the Department of Faculty Surgery (see photo) has been awarded diplomas of the Military Medical Academy for the past 5 years (in 2012-2013 and 2015-2016), took 2nd place in the academy in 2013-2014 and 3rd place in 2011-2012. The average number of circle members was 20-25 people. During this time, 32 reports were made at conferences, including 5 with international participation. 93 publications were published, 18 competitive works (4 of which won prizes at the academy). One work became the winner of the competition for the best scientific work of the Ministry of Defense of the Russian Federation.

7. Treatment and diagnostic activities

  • For 35 years, the staff of the clinic of faculty surgery has been successfully treating gastric and duodenal ulcers. Numerous fundamental scientific works on this problem have been universally recognized in our country.
  • One of the priority areas started by S.P. Fedorov, is hepatobiliary surgery. The clinic uses all new and advanced technologies and methods of diseases of the liver and biliary tract in a planned and urgent manner.The continuation of the glorious traditions of the clinic is the successful development of the problem of surgical diseases of the endocrine organs, which allows a number of leading domestic endocrinological centers.
  • Over the past 10 years, great successes have been achieved in endovideosurgery of the abdominal organs and retroperitoneal space. A number of surgical interventions were for the first time in Russia by the specialists of the clinic.
  • In addition to traditionally priority scientific areas, the clinic develops the issues of coloproctology – the study of pathophysiological and pathomorphological characteristics, artificial nutrition of surgical patients, the whole spectrum of treatment of patients with benign diseases and tumor lesion of the colon, the use of modern physical methods of dissection, and the use of cancer diagnostics in the diagnosis of cancer metastases stomach and colon, introducing the protocol of accelerated recovery after abdominal surgical operations.
  • Every year, over 2000 patients receive highly qualified treatment in the clinic of faculty surgery named after S.P. Fedorov.
